Good gateway cities are Toronto, Cancun, Panama. Nassau used to be, but I am not sure if it still is.

There are no ferries to Cuba. There were cruise liners, but they have reduced since Pullmantur were bought by a US company.

The thorntree on lonely planet has a running joke about ferries to Cuba.
