It's hard to say as it varies from person to person. If this person is doing completely unreasonable things that are uncharacteristic of them--then maybe they are.

There are all kinds of mental diseases. Some mentally ill people are unable to carry on a normal conversation. Some seem perfectly normal but are out of touch with reality--they may believe they're someone else or strange things like that the CIA planted a radio transmitter in their skull. Some are very passive, they just want to sit and watch the world go by, while some are very active, even violent, and can't sit still.

The problem with mental illness is that it's not always easy to diagnose. It's possible for a person to be severely mentally ill but act and seem normal much of the time. It's possible for a person to seem very strange and weird but be perfectly healthy and normal.
