At around 27 weeks they can feel your abdomen and tell you what position your child is in.
Next time you go to the doctor ask him/her to tell you if he/she is head down or not.
Some babies will become heads down just a few weeks before delivery and some never turn. You can have them turn the baby while it is still in your tummy, but it's VERY painful. My friend Brandi had to get them to do this, but halfway through the process she was in so much pain she made them do a c-section on her.
It's too early to really know for sure on if they will turn or not. But mine was heads down for the majority of my pregnancy. They rechecked at 27 and 34 weeks, he never turned again.
Medicaid will pay for another if you ask the doctor to check again for birth defects because of heredity. Doctor's and medicaid wont deny a check if there is a possiblity of birth defect. So if you have to, come up with something.
I was on medicaid and I was terrifed my child was going to be around 9 -10 lbs (I'm a small woman), my doctor gave me another ultrasound and I'm glad I got one, he ended being born at almost 9 lbs. My doctor came up with a possiblity of birth defect plee so that I wouldn't have to pay for it. So trust me, it WORKS.

As a midwife I can tell from abdominal palpation whether the baby is head down or breech. I can also tell from vaginal examination. Midwives in the U.K provide most pregnancy care, only refering to an obstetrician if we find a deviation from normal. Your Dr will be able to do the same. As for delivery without drugs, go for it. Your body is made to have babies, I deliver lots of my mums without drugs and they cope really well. A great book to read is 'Spiritual Midwifery' by Ina May Gaskin, she delivers babies on a place called the Farm in America. All the women deliver naturally without meds and women come from all over to deliver there. It is a real inspiration. Good Luck.

The doctors can feel the baby by pressing on your abdomen. You can too, if you try. Press around and the one spot that feels like a hard knot, is usually the baby. As he or she develops the doctors can feel the different parts of your baby's body. (head, torso, and legs) Remember they went to school, so they can tell a lot easier then we can. As you get close to your due date, they can tell if your baby has dropped into your lower abdomen and if he/she is head down or not. If not your doctor will discuss the possibility of a c-section.
I personally think that it is great that you want to deliver naturally. It is really painful, but it usually makes labor go a lot faster and easier. If you do decide to go natural, remember to breathe...patterned breathing is not required, but you have to force yourself to relax your muscles so that the contractions can do what they are supposed to. If you hold your breath and clinch, it will take longer for labor to progress.

in most cases they can tell by feeling if the baby is in the right position to be delivered vaginally. no worries.

They can tell when they examine you. It all feels the same to me, but doctors can feel your internal organs when they exam you by feeling your abdomen. They can tell if a baby is head down by feeling the outside of your abdomen.
