In the movie Teen wolf when they are at the basketball game towards the end of the movie and all the people are coming off the stands some dude in the stands had whipped out his winkie ya know his little man and when they edited the movie no one caught it
also in the movie three man and a baby everyone said that in a certian scene when the grandma walks thru a room with the baby you could see a ghost in the closet and it was actually a cardboard cut out of one of the actors that slipped thru editing

Star Wars has a couple good ones.
In one scene, a bunch of Stormtroopers are walking down a hall and through an arch. The tall one on the right hits his head on the archway so hard it almost knocks his helmet off. Even after several reproductions of the film for Casette, Laser Disc, and DVD, the scene stayed. Finally, in the new Special Editions they addressed the problem by making the "lead" trooper turn and say "See to him."
Also, during the duel between Obi-Wan and Vader, the angle of the shot causes Obi-Wan's lightsaber turn into a stick with a light at the end.

In the movie Pulp Fiction when Vincent and Jules break into the room with guns drawn (after the notorious "Le Royale" scene in the car) a guy jumps out of anopther room with a long barreled revolver and shous "Die MFs ) while emptying the revolver. Hole appear in the wall behind Vincent and Jules.
Only problem is, in some of the previous shots in the scene, the holes are there already. In some, they are not. An obvious continuity problem.

Commando with Arnold Schwarzenegger has to be one of the worst.
The Porsche is all smashed up but when he pushes it over and drives away there is not a scratch on it.
He attacks the island to get his daughter back. When he blows up some of the buildings the guards only fall over halfway (Cardboard cutouts obviously) and when he throws a gernade you can see the springboards that throw the bad guys into the air.
There is also a good one in the original Star Wars movie. After Luke blows up the Death Star he returns for a hero's welcome. Princess Leia runs up to him and calls "Luke!" and he responds "Carrie!" (Carrie Fisher was the actresses real name)

In "The Point of No Return" (the American remake of La Femme Nikita, starring Bridget Fonda) the guy says her cemetery plot is something like "plot 39, row 12" then a minute later he emphasizes it but states "plot 12, row 39". This drives me nuts!

In Drumline.....some of the band members are at a dinner eating and the one guy is eating his burger really fast, well in the different pictures they show him, the first time he has eaten out of the burger, then second time its a whole burger and when he throughs it down, it only has one bite when he has been mowing on it.
