Do you respond by actually talking about an event of your day?
Is it ok to say that your day was bad?
Is it always necessary to ask how they are?
Or should I/we just stick with 'nothin' or not much?

It depends on who it is, if you don't really care to speak or don't like that person then say nothing much. But if it's someone you like then ask how they are.

You could respond by telling them about an event of your day or you could talk about what is happening in your life. If you do that, maybe they won't ask again. I would ask how they are. They are probably wanting to communicate with you. You should tell them what you want to tell them. I wouldn't say nothing or not much. Tell them what you are going to do that day. What's up is really a term that means what are you doing? , How are you? It is a term people use to get one talking.
