They are about 5 years old and have seemed to be growing ok. But lately they are dying. I keep them watered and fertilize them a few times a year.

How tall is your bush? If your rhody is matured enough, you won't need to water so much. Too much water, or too much fertilizer cause the disease on the bush. How this bush looks now? Is it covered with white stuff? Are the buds get brown and the branches die back? Were there any bugs around you noticed? Are the leaves look brownish? It is hard to say what is wrong with your bush without anymore info... Do you know any mastergardeners in your area? Perhaps, you can take a picture, or take a small cutting from it and show it to them or someone at the local nursery.

Rhododendron's need acidic soil. If your soil is suitable then you may have vine weevil eating the roots. I lost my dwarf rhodi's to vine weevil when I had a courtyard garden a few years ago.

Fertilize every 3-4 weeks. Check for grubs in the soil.

Does it need to be watered? If it does water slowly because it sounds like it is maybe dried out.
