This is easy. Clock Tower on the SNES. Trust me, it creates more tension than any of the Silent Hill games ever could. Also, The Suffering is a good creepy one.

There's a PC game my firend have that they (and the maker's of the game) are convinced is haunted. But I don't know what it is called.
The scariest game I've played, by far, is Eternal Darkness: Sanity's Requiem.

I found the resident evil series to be scary, could be because my worst nightmare would be the dead coming back to attack the living.
