what are the procedures to change? - This will vary by court. If you have already found a new lawyer he can work the process.
Meaning are there any motions to be filed, is there a time restriction on such things? - There is normally a motion filed to make the change.
Why would the judge deny this change? - If he feels that the change is unnecessary and will create a disturbance to the judicial process.

need a little bite more facts, but in general you are free to terminate one lawyer and hire another, it does not mean the old one will not have to be paid for their services

there can be exceptions like if you are currently in a full trial and which to change the judge may reject a motion for more time to fine a new lawyer then you are stuck with the old one

You can fire your attorney at any time. If it is a court appointed one the you must ask approval and have a valid reason.
