I don't because I don't have anything that burns fuel. If you have any appliance that burns fuel, oil, wood, etc you most definitely should have a co detector. Pay the price and get the best one that you can.

I have three. They are expensive so I didn't have them for a long time until I saw some for $5 in a closeout at Wal-Mart. They are plug-in and have a backup battery. If I was going to get just one, I would probably put it near the bedrooms.

Yes, of course. One at each end of the house along with fire/smoke detectors and an additional one near the furnace.
