Depends on the terms of the lease. Typically, it is every month.
I own a couple of homes near the university of Wisconsin-Milwaukee and my lease calls for 2 payments; 1 at move-in prior to start of fall semester and the second payment prior to start of spring semester.

Normally yes. You can pay it annually, in advance, but that's unusual especially for residential property.

Yes or some pay it all up front! You have to pay so that the owner can pay their mortgage and so on.
