For the gold the silver medalist...for the bronze the 4th place finisher...but one silver medalist had to give hers back too so the bronze medalist gets the gold and the 4th and 5th get the others.

The USOC has the medals and will return them to the IOC who will redistribute the medals to the non-drug using finishers, at least the ones that did not get caught.

Whoever was next in line behind her. The second place winner will become first and so on. You just eliminate her from the races and use the finishing order to award the medals.
