Depends.

In most models which use a normal laptop hard disk you just have to unscrew the bottom panel covering the hard disk, take the old one out and put the new one in and then put the panel back on. The manual that came with the computer should tell you what to do.

Though if you've got an ultraportable you might find the hard disk hard to upgrade because normal laptop hard disks aren't used (instead they use small drives).

The difficulty depends on your bravery. Replacing a hard drive in many toshibas requires some disassembly of the machine. as stated above, some times its available from the bottom, other you have to go in under the keyboard. the tricky part is that sometimes companys will glue their own brackets onto the hard drive which must be used to mount in the laptop but is difficult to remove from the old hard drive. If you want a technician to install it for you it can cost on the high end of most rates, usually around 100 dollars in labour alone. The cost of the drive itself will depend on its size but you will probably spend around 150 dollars on the drive.

Depends on model. Some are under a cover on the bottom and a couple of screws hold the frame. Others are under the keyboard that has to be removed. Cost is as low as $50 and as much as $150 depending on style, interface and size.
