I have always felt the doing something for someone is much better then giving them something. It's easy to go to a store and fork out money for something that someone may never use.
Take her to the spot where you got engaged, have a fancy picnic, clean the house from top to bottom, take the kids for the day, sent her up for a day at a spa, fix everything on your "to do list". Sure get her something, but it doesn't have to be expensive.
If you do something romantic, it will be a cherished memory, and you can't buy that in a store.

The answer to this question really depends on several things. First thing is your age. Are you young lovers? Been married for a while? Second thing is your love life. Do you and your wife have a good love life or not? Are you both able to enjoy each other sexually?
Sure, you can take her out to dinner, a picnic, movie, dancing, whatever. But they way you handle yourselves when you are having sex tells your spouse so much about you and the way you feel about them. You know what gets your wife in the mood for sex. Create a situation made specifically for that. If she likes scented candles, place them all over the house. If she likes roses, buy her a dozen, and spread the petals of another dozen all over the bed or in a warm bath. If she likes massages, get some massage oil (with her favorite scent) and after her bath give her a massage. Not just her shoulders, but from head to toe, even rubbing her hands and fingers. Just get her to relax like she has never done before. While you are giving the massage, kiss her every once in a while. You dont have to kiss her on the mouth. You can kiss her in the center of her back, along the spine. Most women find this as an erogenous zone. I bet she will shiver a little when you do this. I think you get the point, and you can use your imagination from here.
If you and your wife dont have a good sexual relationship, maybe you should just get her something she wants. I am sure that she has dropped a hint or two as the day gets closer.

How about a local Spa gift certificate? Jewelry and flowers are always welcome, though. A small box of very good chocolates. If you gave a spa or clothing store or any kind of gift certificate, you could put it in with the chocolate, and have both tangible and intangible gifts. How about a pair of tickets to a play or concert?

(big city) Rent a limo and and taker her to the Opera! -maybe include some close friends.
(country) Get a sitter. Set up a table on the front/back porch put some candles out, nice dinner & wine. Red and white check linen. Yellow daisys or her favorite colors and flowers. Let her know that you know her. Enjoy the nature.

Most women would like these:
1)trip to a day spa (she can get a facial, massage, etc)
2)romantic and/or relaxing weekend get-away
Other than that, depends on what she likes. I'd like tickets to a play or the symphony--but some women would rather be drinking beer watching football than sitting through Liszt or Dvozak...

I liked whoever said a heartfelt letter. That would mean more to me than anything. Pour your heart out to her, tell her what she means to you. But don't stop there. Have the house clean spotless with her favorite dinner ready for her and have it by candlelit. Do the flowers or atleast the petals scattered on the table or in a hot bath waiting for her. Don't expect sex, if she wants to great but let her initiate it.

Make her up really cute little COUPONS.... for things like---one free GARAGE CLEANING or a MONTHS worth of DOING DISHES, or something that SHE usually does that YOU could and WOULD do.... AND, since that's a FREE "gift", you COULD also get her a certificate for an entire weekend at a SPA or maybe buy tickets for you AND her for a romantic weekend somewhere that she REALLY LOVES...*and you might not have been there yet.

the first thing is you have to know your wife that helps.
my wife like thing that i have to think about.. she hate stuff that i can just go in and buy.
she want my time i love to work in wood and have made her many things that she loves. i have planned a weekend away with spa time for both of us. NO phones or work. just us. she love camping and have done this as well.. she love flower but hates bought ones.. so i make her a flower garden that i take care of and take in flowers to her that she put all over the house. she hate gardening. LOL.. i made her jewelry by using a rock drum to do my rocks and then picking out what i knew she wanted.
she love pictures of family and Friends and pets. so one year i had every one take pictures of each other and i took pictures of places we love to go to and our pets. and i put them in a photo album for her we look at it often. i know that there are a lot of woman that want jewelry and flowers and just stuff.. i lucky i have a wife that want me and MY TIME.

A bed and breakfast at a romantic get away with a bottle of wine is a nice surprise.
Go out and have coffee with her and talk about her over a cup of java - perhaps buy her a mug with a saying on it she likes.
Hope this helps.
