what sort of format is the AAMT (medical transcription) certificate exam paper like?

2. Transcription
Each has a seperate fee

Sounds like you're only applying to on-line jobs. If you have no transcription experience, you're not going to get a job working at home. You need to apply at every hospital, doctor's office, dentist, etc. near you. And don't just apply for transcription. Apply for file clerk, secretary, etc, to get your foot in the door.
