The hospital is set in america 4 sure but it is really hard to tell were exactly I'm guessing Washington state somewhere or maybe Oregon because on the episode "My Day At The Races" when J.D. was in the swiming portion of the triathalon, he thought in his head :"As I side-stroked through the salty waters of the great Pacific -- pick an apple, put it in your pocket; pick an apple, put it in your pocket". and also Kim transfered or was doing a fellowship at a hospital in tacoma washington and in the episode "my road to nowhere" they go on a road trip so J.D. can see the baby's first ultrasound.
