The furnace fans comes on and off from the temperature of the furnace by using a thermocouple inside the furnace. If your fan runs all the time, first check your thermostat to see if it has a "fan" option. If its on this setting it will run all the time. If not, there is something wrong with your furnace and you'll need to call a qualified technician.

No it does not have to run all the time although there are advantages to having it run. It will make your electric bill higher. The amount depends on the type of furnace you have. For more info check out the furnace and air quality pages at my source.

Your furnace fan should run when the furnace is heating. Once it cools to a certain temp it should shut down. If it is running all the time there is something in the system that is not working correctly.

If it is a commercial system, it may be designed to run constantly. If it is just a home unit, then, NO... it doesn't need to run all the time.
