Tylenol PM contains two ingredients. One is acetaminophen (its US name; in UK and commonwealth countries, it's paracetamol) and diphenhydramine. You may know the latter ingredient by the brand name Benadryl. Acetaminophen is not toxic, but a metabolite, known as NAPA, is. It uses stores of glutathione. If you take enough, it'll use up your glutathione and injure your liver. The minimal dose needed for acute toxicity is about 150 mg/kg, or about 15 or 20 pills. There's a certain safety factor built into that figure, so triple that to be certain of toxicity. Liver failure takes weeks, months, or even years of real misery to end in death. Diphenhydramine causes sedation and in theory can cause respiratory depression in huge doses and the consequent typical overdose death. In practice, that rarely if ever happens. What is seen is an anticholinergic toxidrome (and I'd suggest you google that term). In little kids, seizures are more common than you'd expect from your average anticholinergic. There simply is no case in which the overdose of this combination has the effect the suicidal often seem to expect, in which they go to sleep and don't wake up.
