I'm not aware that there are five plates that form the Earth's surface. The lithosphere is made out of the upper layers of plates, which fit around the Earth like a bunch of puzzle pieces. The asthenosphere, the bottom layers of the plates, carries the lithosphere during movement. Unless you're talking about what makes up the Earth, which is the atmosphere, crust, mantle, and lastly, the outer core and the inner core.

There are more than five. The largest are the Pacific Plate, North American Plate, South American Plate, Australian Plate and Eurasian Plate, African Plate, and Antarctic Plate. Smaller ones include the Nazca, Cocos, and Caribbean plates (among others).
