Donnie Darko is a qualified cult hit, so it sort of could be considered a sleeper.

S.O.B. had trouble in the theaters and has received scant praise, but it isn't exactly a "hidden" movie.

My choices of "sleepers" haven't been aired for a very long time and haven't been available on even VHS, let alone DVD. However, somewhere, someone might have finally released one or more:

Billy Elliot, about an 11 year old coal miner's son who wants to be a ballet dancer. It's a beautiful, heart-warming, and funny film that didn't get much recognition from American audiences.

A movie called "S.O.B." It was made by Blake Edwards, and starred some pretty big names, including his wife Julie Andrews. In fact, Julie Andrews actually does a topless scene (very brief), but you see her "assets".
It was also William Holden's last film, I believe.
