Does it make me an unfit person to be in social work, teaching, working with kids, etc.?

I don't know if it's on your cirminal record, considering you didn't give any details as to your freak out. But I'd say your experience would make you more valuable in the areas of social work and teaching. You know what it's like to be on the other end of the stick, so in dealing with those who need help you'd be more knowledgeable on how to approach their situations.

If you never went to court and you were never arrested, then it is unlikely that this is on your criminal record. But keep in mind that the applications for those jobs also ask you if you've ever had a history of mental illness. If you lie on your application, you could lose your job if the employer ever finds out.

No, it does not make you unfit. Especially if you have completed a therapy program, or are able to document ongoing therapy (with or without medication!).
It is only on your criminal record if you were actually prosecuted for it. If you went to court over it, yes it is. If not, then no it is not.

If the suicide attempt was the only thing you did at 15, then I highly doubt it would show up on your record. Suicide, although it is stupid and irreversible decision that affects the lives of those around you, is not against the law.
