Do they get paid upfront and maybe a small percentage every time it plays? Or just upfront? Know of any websites with average pay out for a commercial?

There is no average, there is a minimum though and you can get that info from AFTRA. As far as being paid upfront, thats called a buy out and most well-known actors or athletes wouldn't go for that type of contract. Residuals can be one's bread and butter. You do get an initial payment upfront, but typically every time a commercial plays you get paid, but the amount goes down each time and it's also generally less in different markets. It's complicated which is why I refer you to AFTRA.

There is no average, what a star gets for a commercial depends on their own marketability. A very well known athlete for example may not be able to act but will get far more than an unknown actor

It all boils down to what consumers are willing to pay for. The reason athletes and celebs get paid so much is because we, the public, are dumb enough to pay ridiculous amounts of money to be entertained by them.
