How do you determine who is at fault? And what if it can be shown that you are incorrect?

My philosophy in life is not to blame others. Blaming is getting rid of the problem and escaping from it. You must not blame others for you designed your own life. You ought to lead the way to achieve the goals in life. Blamiing is a weak attitude of a person. It draws the person away from reality. Be postive in life for every problem there is a solution. Let us say the damage has been done but we must not surrender but rather stand up and move to fight a good life.

The shame and blame game is a game with no winners. There's always more than enough of the smelly brown stuff to go around and no one comes up sweet smelling. Blame is something people only engage in after they've forgotten how to be responsible and are anxious to fix responsibility on someone in the form of blame, rather than working at what may be a common problem for which they bear at least partial guilt (a more productive form of responsibility which can be objectively evaluated and remedied).

We live in an age where no one is held accountable for even the most blatant and outrageous transgression. No one is to blame, we all share in the responsibility ...

Without a culture of consequence the focus on solutions is a symptom resolution approach, the fundamental issues (the person to blame) remains in tact and continues to create more problems.

The difference of when to confront (blame) someone and when not to is their intent. If it is:

1. Ignorance of the effect of their action. Then explain and show them the result.

2. Incompetence and inability to do it correctly. The teach them how.

3. Indifference to the outcome. Then BLAME.

Philosophy is a shared pursuit of the truth. What you want here are opinions.
Who said it, "There is your side, my side then there is the truth."
I think an outside opinion is needed to find out who is at fault but I don't see the value in blame.
Clean your side of the street, that is all you can do. Often you are both at fault to some degree.

It is worthless, it only serves to make someone feel superior, who has definitely made their share of mistakes. Why not focus on the solution instead of the problem.
