Make contact with the clerk of the court in your area. The clerk will have the required forms as well as the instructions on how to complete and file the documents. The issue of child custody is very complex and unless both parties are in agreement and it is a non issue other than filing the requisite paper work in court, then I would really suggest that you find yourself an attorney in which case the attorney will file the petition for you.

Go to the courthouse and obtain the forms to petition the court for modification. You would be better served to hire an attorney but are free to represent yourself.
