I suggest that you go to the following link. http://www.acupuncture.com/statelaws/stelaws.htm This link provides all of the state requirements for the practice of acupuncture. You will find some states have no laws and that some states allow Md's to practice without any training. Before I report this case I would verify that your state requires licensure.

Does acupuncture require a license? What state?
Nudity is not a crime nor offensive to many people.
Does acupuncture require nudity? Not all of it.
Are they running a business in a non-business area with lots of traffic? Call city code enforcement or state licensing board.

You should weigh up the circumstances and the consequences. The circumstances would be things like whether they have license pending and their general attitude, the consequences would be what will be gained by either speaking to them first or reporting the matter.
Already you mentioned a little kid seeing things it should not, and this relates to bad circumstances.Should you decide to speak to them first, their attitude should guide you further.

I would be the most concerned about the child. The non license thing would not really bother me, but I would report about the child. And like someone else said, I would report anonymously

Mind your business. I'm not trying to be rude, but that's what you should do. It's not your responsibility to decide how people want to spend their money
