A: not particularly relevant. Either way will do just fine.
B: depends on the country! In Europe for example, in most countries it is ok and almost necessary to put your photo. In the U.S. is basically illegal, instead. I don't know where you are.
C: yes. You definitely should review and modify your resume to target the specific employer that will receive it. This is important.
D: I would not encourage that. Nothing wrong per se with it but at the very least it will come across as extremely unusual and raise all sorts of questions you don't need.
