I have the SS-5 form filled out, I also have my certified copy of the marriage license to put with it. What other copies of stuff do I need to make to put with it all to mail to the social security office? We live too far away from one for me to actually travel there and present my stuff in person. So, I have the SS-5, the certified copy of the marriage license, and what else do I need to enclose in the envelope to mail? Do I need a copy of my drivers license or anything in there with the marriage license?

Ive had to get new ones a few times.
You need a certified copy of your marriage license, a certified copy of your birth certificate, and your drivers license.

What social security require is first you change your DL or ID, send a copy of your ID or DL together with a copy of your Marriage certificate and they would change your name. Its not complicated unless you are illegal, they would request to have more information otherwise they above things are the one needed. Copies of your DL, marriage certificate, your previous social security card.

1)u need orinigal document..such as marriage certificte.(no copy, original only)
2) drive licence..
3)bring to the local office SSN near u
4) take abour 1 ot 2 hrs..
