Depends on what facet of cruel you're talking about.

Physically, I would say making someone a quadripeligic would be about as cold as it gets. At least if a person dies, they're not going to suffer with loss of mobility and phantom pains.

Emotionally, I would go with taking away a person's self-esteem. When it comes down to it, self-esteem is the most difficult part of a person's psyche to nurture and grow. Self-confidence is easy to fake, as is a cheerful attitude. You can always tell when someone has low self-esteem, however.

Behind bedroom doors, my money's on the money shot preceded or followed by A2M. Money shots are bad enough by themselves, but add in the other part and it's just downright disgusting. Maximum Security Prisons are hellholes on earth and male on male rape is very common there. These social cesspools hold the worst criminals and the people with the worst behavior on record. They are predators that prefer to prey on humans than to earn an honest living. They turn into predators on each other in jail and it is a dominance game with the biggest, baldest one rising to the top.

Rarely, is rape a sexual crime; it is an intimate crime on the body meant to dominate and ruin the soul. It has no redeeming value, it cannot be condoned by any society and it is one of the cruelest ways a person can behave toward another, a method that is always a crime.

Torture is a wide, wide field and it can be done with brutal precision as well as with neglect. It is too broad of a field to label it as the most cruelest action; but rape is very specific and the ultimate personal act. Even professional torturers don't resort to rape; it is too intimate and too criminal.

Rape breaks a trust in your fellow person or in an intimate friend, it is worse than cheating because the goal is to punish the victim and to reduce them to the status of a sex toy; it is the ultimate loss of diginity.
