The first thing I would look at is the choke. its possible that the choke vacuum dashpot isnt opening the choke plate after it starts. Remove the air cleaner cover and verify that the choke comes open a little bit (about an 1/8" ) when you start it. If it doesnt the vacuum dashpot diaphram is likely ruptured. Fairly easy to replace it if it is bad. By holding the throttle down you are overcoming some of the tension on the choke spring by high air flow into the engine. Dont overhaul the carb until you check this simple operation.

Sounds like a classic case of the carburetor's idle circuit being clogged up. The engine runs off idle, but as soon as the throttle plate in the bottom of the carburetor goes closed, the idle passages come into play and the engine stalls. You will have to remove the carburetor and have it rebuilt. You could buy a rebuild kit and do it yourself, but sometimes it's hard to get the idle circuits flowing right. That's the most sensitive mode the carburetor operates in because only a small amount of air and fuel is mixed compared to the power mode where lots of air and fuel are being consumed by the engine. Make sure to use Stabil gas stabilizer in the fuel tank to help keep the gas from gumming up the carburetor etc.

You could consider buying a direct replacement Edelbrock or Holley carburetor.
When you remove the carburetor you'll probably see a lot of carbon deposit buildup at the base of the carburetor, so it's a good time to clean all the passageways out. You could even choose to replace the intake manifold with a performance Edelbrock manifold that matches their carburetor. There was an exhaust passage way through the intake manifold and under the carburetor that would get clogged up sometimes in the OEM intake manifold and cause poor cold starting performance.

It is none of the things you named. I would look closely for a vacuum or air leak. Check the linkage on the carb. the idle adjusting screw may have moved. Don't turn any screws on the carb itself. Make sure the mounting bolts on the base of the carb. are tight. If you have a vacuum brake booster the hose that goes to the manifold could be leaking. Look closely. I would not rebuild a carb. for a minor idle problem.
