What do I call this form of Letter?

My ex-boyfriend has a number of my belongings at his house and I want them back, what is the letter that informs the person that I want my stuff back?

If it is a letter from you it is a letter of request. Requesting the return of your items.
Also you can use a letter of intent which would inform your ex what you intend to do if such items are not returned.
