No, just take your drivers license and your birth certificate down to the SS office and they will get you a new one. Not sure if they still send them via mail or not. I havent gotten one replaced since 1989

no its easy if you have two other forms of i.d.
even a school transcript or a baptismal record will do
then they will issue you another copy
for a small fee of course
