slide? or break? well it depends on the age of the condom. don't ever keep an unused condom for longer than one month after purchasing it. but one out of every 800 condoms has a deficiency.
hope that i helped,

Condoms are only 85% effective at preventing pregnancy. They are best used with a "second line of defense" such as contraceptive jellys or creams, which are inserted into the vagina. This high failure rate is due mostly to improper use. Tearing of the condom by the teeth or fingers, use of lubricants not recommended for use with condoms, and failure to wear the condom with the first sign of errection. Used properly, studies suggest the failure rate is between 1 and 3%.

vary rarely, my class mate had one in her purse and we blew it up fully and it took like 10 ppl steeping on at once to pop it. they only really fail for improper use like having ten ppl step on them. don't use the condoms if they are over a 4 months old. and also make sure they have lubricant b4 u use them. get the expensive ones don't buy like the cvs brand. but they mainly pop during the process of putting them on so just make sure you put them on right.

they will pop every time if you don't put them on right.
sqeeze the tip so np air is in the condom when you put it on. then after you have it rolled all the way down pull at the tip to give yourself a little slack. you need to make a little room for what's going to come.

Condoms have a failure rate of less than 2% and most failures are due to improper use.

1.3% in one study
3.4% slipped off in another study
7.3% broke during application
There are alot of numbers floating around because each test would have its own results.
This is the standard way the tests report "failure"
Nonclinical breakage rate --
number of condoms reported to have broken while opening the package or putting on the condom divided by the number opened.
Clinical breakage rate --
number of condoms reported to have broken during intercourse or withdrawal divided by the number of condoms used during intercourse.
Total breakage rate --
number of condoms reported to have broken (nonclinical and clinical) divided by the number of condoms opened.
Complete slippage rate --
number of condoms reported to have slipped completely off the penis during intercourse or withdrawal divided by the number of condoms used during intercourse (if participants report a clinical break and a complete slip, only the clinical break is counted).
Total clinical failure rate --
number of condoms reported to have broken during intercourse or withdrawal, or slipped off completely divided by the number used during intercourse.
Total failure rate --
number of condoms that have broken (nonclinical and clinical) or slipped off completely divided by the number opened.
this was just what i learned from the two sources listed below that i found by doing a search for "condom failure"
