Besides the low birth weight that was mentioned previously, the baby could be born with asthma. I've also heard that there's a correlation between SIDS (Sudden Infant Death Syndrome) and smoking. Too, the baby may grow up to smoke. Smoking probably induces a lot of other diseases as well that I'm not familiar with.
She should quit and you should encourage your friend as much as you can.

People should never smoke when pregnant.
1. Babies are being born with chronic asthma, like my daughter she is 11 years old and have asthma, because I was to selfish to quit smoking.
2. organs do not fully develope.
3 Ask the doctor for a pamlet on birth defects.
3. Slow learning.

Worst case scenerio:
Baby is underweight, comes early, has asthma, and has IQ lower than it would had it not get it's brain coated with tars and creasote from the 2nd hand smoke, placenta is abnormally attached, and either comes away to soon or doesn't come at all and needs to be surgically removed, or it covers the cervix, necessitating surgical delivery of the baby, and mom hemorrhages because smoking interferes with vit. C absorption. Then baby dies of SIDS.
Although these things rarely happen, they are all more common in smokers!

The worst can happened? Let see by Hollywood standards. Fall asleep while smoking set the apartment complex on fire and hundreds of people including her and the baby die, which in turn infuriates people and start a riot (and more people die), then the son of an ambassador dies and world war 3 starts and billions die. So to save the world she need to stop smoking!
What the &%^$, smoking reduces blood flow which is bad for ANYONE! 2nd hand smoke is bad for people near her. How about stop smoking and save money, keep clothes smelling better, teeth whiter, better health, better health for the child, and the health of people near her.

That depends on what you define as worst. The child could die. There could be birth defects, some severe, premature birth, or developmental problems if the child is actually physically ok. Of course, it's possible that the kid would be fine. Lots of them are. However, smoking while pregnant is essentially child abuse: you are knowingly doing an action that could harm your child. If she will not stop, you should consider reporting her to child welfare.

My boyfriend has a daughter and his ex smoked the whole time she was preg. There baby is healthy though she was born with a cyst on her head. But I would never suggest you let her keep smoking. Thats just one case out of a million. I wouldn't take that risk any day.

Well First off her baby will be addicted to nicotine at birth and babies like that tend be much fussier. She could also end up giving her baby a birth defect. One of my friends smoked the entire pregnancy and her baby ended up having a cleft palette. It's not worth the risk. This is a precious innocent baby who did not ask to be here. Personally I don't see how you could inflict that on your child.

Low birth weight, birth defects (ranging in severity and complication), or nothing.................. just a chance a woman takes.
I wouldn't recommend it. And, I'm a former smoker, and 37 weeks along with my son. It was hard as hell to quit, but I did it for my son.
Edit: OH... just to let you know... the placenta is wonderfully designed to stop harmful things from reaching the baby, that's it's job. But, studies show that the placenta can not stop nicotine, alcohol, or any other drug, and so it's something that the baby directly absorbs through your bloodstream by the umbilical cord. Just food for thought.

many women who smoke have premature babies as well as low birthweight babies even if they aren't premature but that doesn't seem the worse to me.
recent studies show that children of women who smoked during pregnancy and childhood tend to be smokers when they get older, so a woman may very well be condemning her child to a lifetime habit that is nasty and hard to quit.
another study shows that babies of women who smoke during pregnancy tend to have fertility problems when they get older. i wonder if that is why so many people these days have so many fertility issues??? back in the day nobody thought anything about lighting up and puffing away.
