My teacher thought I was smart, but I made a lot of dumb mistakes.... I feel horrible.... It makes me not want to do my work eventhough I know I will......

Your teacher was right. Chill. You are probably just choking because you are thinking too much and trying too hard.

What is the question? Go to your teacher after class and ask for help understanding the material. Most schools have homework clubs for out of class help with assignments.
