There are some films from Japan (as well as other countries) which are explicitly gruesome on multiple levels. Audition probably has one of the most disturbing scenes ever, Old Boy is very messed up, and Battle Royale has violence in basically every minute of the film.

House of 1000 Corpses, Planet Terror, Saw movies - I've heard that Hostel is really bad but haven't yet seen it

There's a really nastly bit in Wolf Creek when he stabs her in the spine to paralyse her which is really unpleasant!
