Tell them you need a COMPLETE physical. If they do not understand or if there is something SPECIAL that you want done request it at the time you make the appointment. It is very simple.

Just call and get an appoinment and when you get there tell the doctor that you want to get a full body checkup.

Visit or call a doctor, tell him your specific problem, if any, or else get the Lab's name for general check up for body, blood, urine, cholesterol, sugar levels.
