I say wash them on the outside everytime they get dirty, or every other time. On the inside, maybe once or twice during their life with you, but be careful, it can ruin them to put them in the washer.

Whenever they get dirty....as in you can see the dirt on them.
If they are dress shoes you should shine them every time you wear them which probably isn't too often.
