It depends on the player and the piece of equipment.
Brendan Shanahan has used the same two sets of shouderpads since the mid 1980s (yes...I said 1980s)
Mats Sundin has used 2 helmets his entire career (Mike Foligno and Butch Goring used only one).
Craig Ludwig used the same snow-plow blades as shin pads for his entire career (or am I thinking Gerald Diduck).
Each player gets 2 jerseys a game, and sticks and skates are replaced frequently. The rest of the equipment is replaced as the players see fit.
Players are responsible for their own equipment, whether they buy it, receive it free through sponsorship/promotion, etc is their business.

Most players that I've heard asked about this say they only change their pads under a few select circumstances:
1. They feel like the pads they are wearing are giving them bad luck (mostly goalies).
2. They break or are otherwise damaged beyond repair.
3. They want to keep them because it was worn on a special occassion (certain goal plateau, win mark, etc.)
4. Old equipment broke and they are trying out new stuff until they find something they like.
So to answer your question, they don't change their equipment hardly ever and I'm pretty sure the team pays for it.

My brother played 14 seasons of pro hockey for 11 teams, including 4 NHL teams (including your Devils).
Never did any of his teams buy him his equipment. The players even buy their own gloves, it is their responsibility to make sure the colour scheme is right.

depends on the player...
things like shoulder pads, shin pads, pants, elbow pads rarely get replaced. probably when they are broken
gloves - maybe once a year, maybe more if you are a top notch player getting alot of ice time.
