MacArthur was placed in as "President" of Japan and acted as such. He rebuilt the entire Japanese economy and styled it after ours.
When he was replaced and Japan was given their freedom he was sent to Koren but, then over jealousy and a stubborn President he was recalled home and retired.
MacArthur was the only General that spent his entire career out of the U.S. he retured to the U.S. to retire.

For all of MacArthur's faults he had no desire to be an Emperor, as another answerer noted, MacArthur was happiest being called General, though one does imagine him reveling in the title Generalissimo. Yes MacArthur 'Lorded' it over Japan and enjoyed being the Supreme Commander. MacArthur was very American. Having a snazzy limo driven by a dashing driver, a palatial office, a comfortable home, his own private toilet, a Private chef, those sort of things were all that MacArthur needed to be content oh that and the feeling that he was Master of the World.

For MacArthur's Ego the Japan Gig was good - - - and he 'acted' the part but in truth if one examines his various military commands, again this is germane to America, the system mostly ran on its own and most of MacArthur's Superiors & Immediate Staff 'ran' the show with every show that Mac was doing the work - - - It works that way in Corporate America ....

Wouldn't that be the president of Japan? Since he was the president of the US who invaded Japan so he would keep the same title, right?

No. He'd rather be a General than an Emperor.
