He or she that takes the first swing, fires the first shot, mouths the first insult is the one who initiated the fight. Period. When you initiate a fight then YOU are the loser, no matter who is victorious. You lose because you have run out of good arguments for your cause and have resorted to making "might make right" and shown the weakness of your end of the discussion. A fight ALWAYS shows the weakness of the initiator's position.

In regards to winning the fight, it doesn't really matter who draws first blood so much as it matters who wins in the end. Although if that first blood is a serious injury to the opponent, then it could basically mean they are more or less defeated.
Although from a legal standpoint, it'd be better NOT to draw first blood because then you can claim self defense and have a better chance of convincing people that you're the "good guy".

Physical fight:
YES! Mental advantage is huge if you can successfully injure the opponent first. They will be on the defensive while you can continue to pound away at them. Legal bull aside, if you wait for the first blow you may not be able to get the second, take them out hard and fast. There are several ways that a trained or skilled person can make the first blow the last blow.
However, unless this is really a kill or be killed situation or the like, try going as much as you can with compassion and mercy. If it looks like they are going to get mean with you though, don't let them.

It seems to me no one is truly reading this question. In a fight, Physical or otherwise, it is normally best to be the first to strike. Doing so gives you you the advantage of having the upper hand momentarily, this is why most football teams choose to receive when they win the coin toss, so they can achieve the first goal and demoralize the opposition. Also by "throwing the first punch" you can weaken your opponents ability to strike back. For example in a debate, if you are the first to speak you have the opportunity to address your opponents arguments before they even speak them, provided to can accurately predict those arguments.

John Wayne used to say (and I am sure that this was not original with him) that it was best to never start a fight, but always be the one to finish it.
I don't think who draws the first blood is the most important ... its the one who draws the LAST blood.

It depends on a lot of circumstances. If this is with a partner who you love then absolutely no its not. If you are a man and you get into a confrontation with another man… it might be best to throw the first punch.
It also depends on your attitude and personality. I kind of like the idea of throwing the first punch, bypassing all talking, cutting out all the bullshit. That works for me though, at this point in my life, for you it could be totally different.

Depends on the fight. But generally no, it's not better to come out swinging. As I tell my daughter, resorting to violence means you've run out of intelligent things to say.
Of course this applies to individuals, not countries waging wars.
