I just bought a new product called Leaf-Blocker. It seems to be a system that makes sense, it was about half the cost of all the other systems in the JB Dollar magazine. The salesman who came out selling products other than the hood systems showed me not only icicle problems with the hood systems but also alot of bee and wasp nesting under the hood. I live in North East Ohio so winter weather can get tough.

A mesh screen, cut to twice the width of the gutter. Then cut the length, whether it be 5, 10, 15 ft.
It must be a screen that would pop back straight if you bent it into a half moon shape.
Bend to a half moon and place in the gutter. You now have a canopy over your gutter.
You might want to rivet it into place

Gutter Helmet is used more and more frequently up here. I've even heard from my gutter installer that it is his no.1 pick. as for icicles I have a feeling that most products will have that problem. You might just want to use the roll out mesh that bends upward as you tuck it into the gutter. Its sold all over the hardware circuit

Once a year maintenance,A roofing or gutter company will take cleaning jobs at the end of the season,this cleans all debris from the gutters,give them access to your roofline and allows them to address possible problems you can't see as a homeowner.
I have done this for almost 20 years,advertising gutter cleaning for 29.95-39.95 depending on the access and pitch of the roofs.

The best way to keep your gutters clean is to match up with a qualified contractor in your area that can asses your tree coverage, roof type, water volume and from there, install the correct product for your situation.

Gutter protectors or screens are best unless you wish to hire a service to maintain them. The protectors and screens are fine for northeast winters.

They make pieces that snap over your existing gutters and they work very well. I bought mine at Lowe's and they're easy to install.
