At least in some school districts, the parents/guardians must agree to the recommendation. A friend of mine taught high school in Denver. One semester, she recommended that 20 students repeat the grade. Only one parent agreed so the other 19 were advanced to the next grade even though they didn't have the skills to succeed at that level!

As long as the child is meeting the state guidelines in his or her required subjects they will be allowed to proceed to the next grade. If they are failing during the benchmark years 3rd,6th and 8th they will not promote them to the next grade unless they attend summer school. If they have not met the guidelines in reading and math they will not be promoted to the next grade and will not be allowed to attend summer school. In Chicago it is hard to fight a demotion when a child is failing.A teacher can recommend that a child repeat a grade but that's all it is a recommendation. A parent can recommend that their child repeat a grade but it can get real complicated.A meeting will need to be scheduled and all parties would be involved.Parents, teachers, case workers, psychiatrist and the principle or assistant principle would sit and discuss why the child should or shouldn't be promoted.
