Most of the above answers are right on. Also, the title is derived from an American children's folk rhyme.

Wire, briar, limber-lock
Three geese in a flock
One flew east, one flew west
And one flew over the cuckoo's nest.

In Ken Kesey's book, (from which the movie comes from) he uses the last two lines of the rhyme as the opening.

Because two flying over the cuckoo's nest would have made them too heavy and they would have crashed right into the nest.

properly i'm no genius or something, yet as cuckoo is a slang be conscious for mad, cuckoos nest is a psychological homestead, one flying over it would propose the guy who have been given faraway from the homestead, that would desire to communicate over with countless of the characters.

Because it was about mental patients &Cuckoo is a slang term for that.

The title was symbolic, primarily, great film!
