Members of royal houses have adopted children before. The current heir to the Norwegian throne, Crown Prince Haakon, adopted his wife's son from a previous relationship with no problems. However, children adopted into royal families have no status and are not in the line of succession. This means that little Marius, the son of Crown Princess Mette-Marit of Norway (the wife of Crown Prince Haakon), will not ever be king of Norway.

Yes, but they are unlikley to have the title HRH or be given other titles. In the Peerage of tjhe United Kingdom adopted children have "The honorable" before their names but cannot inherit titles. Christopher Guest and Jamie Lee Curtis (Baron and Lady Haden-Guest) have adopted two children but they will not inherit the Baroncy.

There is no law preventing them from doing so. If this happened, the sovereign could grant a title to the adopted child, but it would not come automatically as it would with a biological child. As others have noted, the child would remain outside the line of succession.

If example if Prince Andrew Duke of York adopted a child him/her would not have a title. Its all to do with blood. The late King of Jordan adopted one children but they never had titles. Abeer Muhaisen, adopted daughter of the late King Hussein of Jordan. But no title.
