a care plan has different parts to it.
1, name and address
2, date of birth
3, name of social worker
4, next of kin,
5,any current illness or past illness including surgery
6, name of gp, dentist
7, how many children
8, whether married,
9,occupation,
10, any allergies
2nd part
1, are they able to wash themselves,
2, feed themselves,
3, attend to toilet needs,
4, are they mobile,
3rd part
1, sleeping habits,
2, waking habits
3, how much assistance will be needed/not needeed
4, bathing habits
4th part
1, activites
2,what things they like to do ie play dominos or bingo
5th part
their health
1, any surgery
2, their medication and what theyre taking it for
also you need to ask the family if the client is for resucsitation and if not you need a written letter signed by them and make sure the gp is aware of it and that it is documented in the care plan and highlighted on the front page

Ask the doctor what they need. If you are paying for their care you can have them get hair cuts or other services more often than other people get them. If a medical condition requires them to have a special diet or they have religious needs for special diets you will want that included.
Also include what to do for medical care if case of sickness or injury.

list wot needs they have...i.e need supervision wen walkin etc. wot thr hobbies r and how you can help to enforce them
r u able to promote independence or..
medication
family contact days and thr info
staff ratio per resident inc. keyworker
..wot things n belongins they have..
it all depends who its fot but mainly a list of every single thing to do with the service user and how u intend to care for them.
include records of their care manager\ social worker.
i cud go on 4 eva but it depends on ur setting

for the financial bit, you could try NHFA - nursing home fees agency. I believe they offer free advice.
