It should be where pineal gland is. You can imagine it to be between the eyebrows on the forehead.

Since the visual cortex of the brain is in the back of the head/skull, it would seem reasonable (with slight physiological modifications) that a third eye would be best situated in that region of the head. There would be evolutionary survival advantages too.

Given that this is an "if" question that does not have any basis in biological natural selection, the answer would be "wherever you want it to be".
Is that 3rd eye supposed to be similar to the other two, and having the same function and characteristics?
