Challenger exploded in mid air on 28 Jan 1986

Columbia destroyed on re entry on 1 Feb 2003.

Apollo 1 burnt on the ground on January 27, 1967.

That wasn't a space shuttle. It was the Apollo I mission, which was supposed to be the first launch of a Saturn 4 rocket on a mission to prepare for the eventual moon shot.

Back then, the astronaut capsule was filled with pure oxygen, and something sparked while the astronauts were still sealed into the capsule and buckled into their seats.

A spark in a pure oxygen environment instantly flashed into a fire, and burned Gus Grissom, Ed White, and Roger Chaffee to death .

After this, NASA learned to fill the capsule with an inert gas.

Two years after this tragedy, NASA succeeded in landing men on the moon.
