Yes, maybe half a dozen times, and in other small shops too. Always a woman, over 40 years old, and very confident in her toughened bare feet. Never a young person.
NOT against OSHA rules!
OSHA rules are clear, and safety equipment in stores does not require foot covers for top or bottom.
Neither do rules for restaurants.
We have a barefoot bar maid at one of our busy full service restaurants, and her boss checked with the health department and with OSHA and they said no problem.

No, it's rare to see another barefooter here in the Netherlands, I've only seen a handful of other barefooters in the ten years I've been going barefoot. The only people I know who work barefoot are some of my colleagues in the history theme park (where it goes well with our period clothing) and one colleague took off his shoes during a hot spell in summer when I worked at an ISP helpdesk, too, but as soon as temperatures were anywhere more normal he put them back on again.
