It can startle a person! Imagine that you look in the mirror as daily routine, and at that instant, instead of seeing yourself, you see someone else's reflection.
You can see this sort of illusion in the movies, where a conversation takes place with a guy looking into a mirror, but his reflection is that of different person talking back at him.
How is it done?
Shoot the scene with a green cover over the mirror. Then remove the mirror and shoot the other half of the scene with the camera where the mirror was and the other character. Then in your NLE, flip the footage and composite it in the mirror.

Go in the bathroom with your friends and shut the lights off it has to be dark. Turn around three times chanting bloody Mary. Then turn on the lights and look in the mirror. There you will see bloody Mary.

I would start questioning my own sanity cos . . . a) what the hell are you doing in my mirror? and b) I haven't had any wacky tobbacky.
