Not personally... yet.

We have some unmanned probes wandering around there, but no people.

The trip is extremely problematic. Getting people there is no problem. Even orbiting the planet is not all that difficult. But so far we've not really "landed" anything on the surface. We've essentially hot a rocket at the planet. The payload was essentially wrapped in bubble wrap so that it didn't splatter when it hit. We slowed it's reentry down enough so that it didn't burn up, and then let it bounce around until it stopped. We can hardly do that with a manned mission. And then there's the problem of taking off. The moon was easy.. practically zero gravity... I think 1/8 that of Earth. We could have fired a .22 at the earth and the bullet would have escaped the moon's gravity. Mars has, I believe about 80% of Earth's gravity. We'll have to have fuel both to land and to take off. Plus we'll have to send enough supplies for the mission to stay a while. Earth and Mars aren't exactly in synchronous orbits.

So, it'll be a while.

No, that's never happened. But there are unmanned rovers on it. But it is the most likely planet to be visited by humans, maybe within a few decades.
