A boat in good working order, a radio with fully charged batteries and enough gas to... ok, ok, with these things I wouldn't be stranded... So:

1) Some means of purifying or being supplied with drinking water Which could be a fancy gadget for purification or could be as simple as some sort of waterproof container to store water.

2) A carbon-bladed knife or hatchet. There are a million reasons why a knife would be necessary. The carbon bladed ones never need sharpening.

3) A small first aid kit that might have an anti-biotic so a simple thing wouldn't wind up killing me, a few aspirin, some penicillin, anti-diarrhea med, pepto-bismol, band-aids, tiny flashlight, etc. I would hope that I would find waterproof matches or a lighter in there too.

A friend, first and foremost. Chances of survival are greatly improved when you are not alone.
Survival Knife, no one should ever be out in the wilderness without one.
Binoculars. Not only can they be used for spotting ships or other things on the island, but they can also be used as a firestarting tool. However, if either me or my buddy wore eyeglasses, I would rather have an ax, than the binoculars.

I'm assuming it is a tropical island, with lots of palm trees. This way I could use the trees to make shelter and live off the coconuts as well.

If the island is tropical, then I would be able to catch rain water.

If I was stranded..well..no boat, eh? A unlimited supply of fresh water, a archery set with steel bows, a unlimited supply of flint.
